Dr. Doneskey is the region’s leading Oral Medicine Specialist, providing consultation and ongoing non-surgical care for snoring, sleep apnea, TMJ disorders, and orofacial pain. These conditions are interrelated, and many patients have more than one.

Snoring and Obstructive Sleep Apnea

Snoring and obstructive sleep apnea are treated with a custom oral appliance for patients who cannot use CPAP and for those who simply prefer an oral device. Because sleep apnea is often associated with nighttime clenching, patients are also evaluated for related TMJ disorders.

Home Sleep Testing

Home sleep testing is available through the office using the WatchPAT device. This device is unique in that it is easy to use and records true sleep – light, deep, and REM. Classic TMJ Disorder

Classic TMJ disorder commonly presents as jaw or facial pain, headaches, and joint pain, and is often mistaken for sinus, ear, or tooth pain. It is best understood as a problem of nighttime clenching and grinding, which occurs as part of normal sleep. Muscle pain and joint inflammation can follow when clenching forces become high enough. Treatment focuses on reducing clenching forces with oral appliance therapy, while supportive measures are used to relieve muscle pain and promote recovery.

TMJ Internal Derangement

Problems within the temporomandibular joint include popping, clicking, and locking, as well as degenerative joint disease of the condyle. These conditions are often associated with nighttime clenching and muscle tightness, making an accurate diagnosis important.

Treatment may include oral appliance therapy to reduce clenching forces, or a repositioning appliance to stabilize the popping. When there is jaw locking, we first restore opening using a jaw mobilization device, then use an oral appliance to protect the joint over the long term.

Trismus and Limited Jaw Opening

Restricted jaw opening can occur following oral surgery, radiation therapy, or repeated dental injections. Treatment focuses on gradually regaining and maintaining normal jaw opening through a structured jaw mobilization program.

Neuropathic Pain Disorders

Trigeminal neuralgia, burning mouth, and other neuropathic pain conditions affecting the face and mouth. Careful diagnosis is essential in determining the appropriate approach to treatment and long-term management.
